Important Forms
Exemption Form:
- Members are excused from only one meeting a semester and members must turn in an exempt form.
- A hard copy must be filled out, signed, approved, and submitted.
- The approval of an exempt form is at the discretion of the executive board.

Volunteer Form:
- Our organization requires 5 volunteer hours for every semester that you are a member.
- Members must fill out the volunteer form and acquire a signature from a supervisor.
